Ambulatory Nurse Specialist

About the Role:
The Ambulatory Registered Nurse functions as a member of the Healthy Aging Transition Services (HATS) care team, conducting duties such as phone triage, patient education, scheduling appointments, referrals, prescription refill, care coordination, patient intake, and other related tasks as needed.
Key Responsibilities:
- Conduct phone triage and provide patient education
- Schedule appointments and referrals
- Manage prescription refills and care coordination
- Perform patient intake and other related tasks
Requirements:
- Graduate of a National League for Nursing (NLN) accredited school of nursing
- Licensed as a Registered Nurse (RN) with a valid license to practice in the state of Indiana
- One year or more of nursing experience required
- Geriatric experience preferred
- Previous medical office/outpatient experience preferred
