Job Summary:

As a Case Manager at Southwest Network, you will play a critical role in supporting adults living with serious mental illness. You will work collaboratively with a clinical team to engage, educate, and support members in achieving their recovery goals.

Key Responsibilities:

  • Develop and implement individualized service plans to meet members' needs and goals
  • Provide supportive services, including assistance with maintaining and modifying covered behavioral health services
  • Partner with members to develop a role for themselves outside of their mental illness, focusing on expected outcomes of increased employment, meaningful community activity, and independent living status

Requirements:

  • Master's or bachelor's degree in a behavioral health or related field
  • Must maintain current CPR, First Aid, and Therapeutic Options certifications
  • Must maintain a valid Arizona driver's license and car insurance
  • Ability to maintain excellent member and family relationships and provide excellent customer service
