Job Summary

As a Case Manager at Southwest Network, you will play a vital role in supporting adults living with serious mental illness. You will work collaboratively with a clinical team to engage, educate, and empower individuals to achieve their goals and improve their overall well-being.

Key Responsibilities
  • Develop and implement individualized service plans to meet the unique needs of each member
  • Provide supportive services, including assistance with daily living tasks, medication management, and connecting members with community resources
  • Collaborate with the clinical team to ensure seamless care coordination and communication
  • Participate in ongoing training and professional development to stay up-to-date on best practices and industry developments
Requirements
  • Master's or bachelor's degree in a behavioral health or related field
  • Bachelor's degree in a non-related field plus one year of related experience
  • Associate degree in any field plus one year of related experience
  • High school diploma or equivalency certificate plus three years of related experience
  • Current CPR, First Aid, and Therapeutic Options certifications
  • Valid Arizona driver's license and car insurance
  • Ability to maintain excellent member and family relationships and provide excellent customer service
