Job Summary:

We are seeking a highly skilled Perinatal Public Health Nurse to join our team at the Georgia Department of Public Health. As a Perinatal Public Health Nurse, you will provide home health visits in Richmond, Emanuel, Jenkins, and Jefferson County, promoting the health and well-being of populations with special needs during disasters/emergencies and biological and chemical protocols.

Responsibilities:

  • Provide professional nursing care in accordance with nurse protocols, policies and/or procedures.
  • Focus on the prevention of illness, injury, or disability, the promotion of health and maintenance of the health of populations.
  • Identify high risk populations and tailor interventions based on the needs of the population and resources available.
  • Provide home visiting services to identify, assess and provide care coordination services to eligible perinatal patients and qualifying infants.
  • Coordinates program services with other service providers and community organizations to promote optimal implementation.
  • Screen and refer clients to other community service providers.
  • Monitoring per program guidelines, educational instruction, materials to promote wellness, safety, and pregnancy and postpartum complications.
  • Monitor for infant complications to include feeding, weight, head circumference, and developmental screening.
  • Develop a plan of care for individuals and families, modify treatment plans as indicated by client's responses and conditions.
  • Provide referral/linkage when medical and environmental risks are identified to appropriate services.
  • Promote program through community engagement and outreach events. Develop and sustain community partnerships.

Requirements:

  • Bachelor's degree in nursing from an accredited college or university AND current Georgia license to practice as a Registered Professional Nurse AND CPR certification.
  • OR associate degree in nursing from an accredited, college or university AND One year of nursing experience AND current Georgia Registered Professional Nurse license AND CPR certification.
  • Must have the ability to work independently, be self-motivated and have exceptional time-management, interpersonal and organizational skills. Maintain knowledge of and seek out new community resources.
  • Have reliable transportation.

Preferred Qualifications:

  • Perinatal and/or infant experience
  • Bilingual, computer skills, and outreach service experience
