Job Summary

We are seeking a skilled Perinatal Public Health Nurse to join our team at the Georgia Department of Public Health. This role will provide home health visits in Richmond, Emanuel, Jenkins, and Jefferson County, promoting the health and well-being of populations in these areas.

Key Responsibilities
  • Provide professional nursing care in accordance with nurse protocols, policies, and procedures.
  • Focus on the prevention of illness, injury, or disability, and the promotion of health and maintenance of the health of populations.
  • Care for populations with special needs during disasters/emergencies and biological and chemical protocols.
  • Identify high-risk populations and tailor interventions based on the needs of the population and resources available.
  • Provide home visiting services to identify, assess, and provide care coordination services to eligible perinatal patients and qualifying infants.
  • Coordinate program services with other service providers and community organizations to promote optimal implementation.
  • Screen and refer clients to other community service providers.
  • Monitor per program guidelines, educational instruction, materials to promote wellness, safety, and pregnancy and postpartum complications.
  • Monitor for infant complications to include feeding, weight, head circumference, and developmental screening.
  • Develop a plan of care for individuals and families, modify treatment plans as indicated by client's responses and conditions.
  • Provide referral/linkage when medical and environmental risks are identified to appropriate services.
  • Promote program through community engagement and outreach events. Develop and sustain community partnerships.
Requirements
  • Bachelor's degree in nursing from an accredited college or university AND current Georgia license to practice as a Registered Professional Nurse AND CPR certification.
  • OR associate degree in nursing from an accredited college or university AND One year of nursing experience AND current Georgia Registered Professional Nurse license AND CPR certification.
  • Must have the ability to work independently, be self-motivated, and have exceptional time-management, interpersonal, and organizational skills.
  • Maintain knowledge of and seek out new community resources.
  • Have reliable transportation.
Preferred Qualifications
  • Perinatal and/or infant experience.
  • Bilingual, computer skills, and outreach service experience.
