Responsibilities:
  • Develop and implement individualized service plans (IPPs) tailored to each client's unique needs and goals, ensuring alignment with agency philosophy, processes, and documentation standards.
  • Regularly monitor the implementation of IPPs, providing comprehensive case documentation and progress reports in accordance with agency and state guidelines.
  • Offer guidance and support to clients, families, guardians, or conservators regarding developmental disabilities, available services, and resources that can enhance their well-being.
  • Assist clients and families in accessing public and community resources, including medical care, educational opportunities, transportation, and social support networks.
  • Collaborate with multidisciplinary teams to assess client needs, develop comprehensive service plans, and ensure seamless coordination of care.
  • Make referrals to qualified vendors and service providers to meet specific client requirements, adhering to agency standards for purchasing services.
  • Conduct monitoring visits to licensed residential facilities and other vendor services utilized by clients on your caseload, ensuring compliance with established agency protocols.
  • Provide direct or indirect advocacy to safeguard the rights of clients, including civil, legal, and administrative protections afforded to individuals with developmental disabilities.
  • Educate clients, families, and community members about developmental disabilities, available resources, and the rights and legal protections of individuals with these conditions.
  • Identify cost-effective or no-cost solutions to meet client needs, promoting independence and self-sufficiency.
  • Participate in agency and external training programs to enhance professional knowledge and stay abreast of best practices in the social services and developmental disabilities fields.
  • Cooperate with public agencies (schools, rehabilitation services, medical providers, etc.) to ensure coordinated service delivery for clients and their families.
  • Maintain strict confidentiality and adhere to all relevant ethical guidelines and regulations.
