Connections Health Solutions is seeking a highly skilled Substance Use Disorder Case Manager to join our team. As a key member of our crisis response team, you will be responsible for managing the care of individuals across both youth and adult populations who are in crisis and stabilizing from crisis situations.
Key Responsibilities:- Facilitate admission intakes for both mental health and SUD populations, educating and informing individuals and their families about consents for treatment and authorizations for use and disclosures.
- Develop and implement individualized, person-centered Treatment Plans suitable for brief intervention.
- Gather collateral information from individuals and involved parties to conduct a thorough assessment of needs for treatment and discharge planning.
- Actively coordinate care and provide referrals to ensure services are aligned with both facility staff and external providers.
- Collaborate with the Interdisciplinary Treatment Team to engage with, monitor, and communicate with individuals, ensuring safety and meeting their needs during the discharge process.
- Maintain accurate clinic records, documenting all activities related to service delivery, including brief interventions, coordination of care, discharge planning, and treatment planning.
- One year as a Drug and Alcohol Case Management Specialist Trainee;
- A Bachelor's degree in Chemical Dependency, Sociology, Social Welfare, Psychology, Nursing, or a related field, and one year direct service experience performing counseling or case management work in alcohol or other drug treatment setting;
- OR a Bachelor's degree which includes or is supplemented by the successful completion of 18 college credits in Sociology, Social Welfare, Psychology, Criminal Justice, or other related social sciences, and one year direct service experience performing counseling or case management work in alcohol or other drug treatment setting.
- Master's degree in behavioral health or related field;
- Licensed Mental Health Professional (LMHP);
- Experience working with children, youth, families, and adults with behavioral health issues;
- Knowledge of and experience with the Pennsylvania State involuntary treatment process;
- Ability to perform Safe Clinch restraint techniques.
