Behavioral Health Case Manager

Job Summary:
We are seeking a skilled Counselor I to join our team at Penn State Health. As a Counselor I, you will play a vital role in providing high-quality care to our patients. Your primary responsibility will be to assist with establishing a diagnosis and a therapy plan, under the supervision of the Physician and clinical team.
Key Responsibilities:
- Assist with gathering and documenting data necessary to implement the therapeutic plan of care.
- Perform diagnostic and therapeutic tasks under the supervision of the Physician and clinical team.
- Collaborate with the clinical team to develop and implement individualized treatment plans.
Requirements:
- Bachelor's Degree in a related field.
- Act 31 training completion required within 45 days of hire.
