Key Responsibilities

  • Accumulate admissions and intake information

Requirements
  • LPN/LVN license in the state of practice or one year of healthcare experience
  • Basic technology skills, including mobile device operation
  • Previous experience with EMR systems and Homecare Homebase (HCHB) is a plus
  • Valid state driver's license and reliable transportation
