Position Overview:
The Clinical Nurse Specialist is responsible for assessing, planning, implementing, and evaluating the nursing care delivered to a designated group of patients.

This role involves leading nursing team members in patient care as directed by the attending physician and in collaboration with other healthcare professionals, adhering to the philosophy and policies of ProMedica Acute Care Hospitals.

Actively contributes to creating an environment that enhances satisfaction for patients, families, physicians, and staff.

Ensures that decisions are made based on a patient and family-centered care philosophy, utilizing evidence-based practices with a focus on safety, customer satisfaction, and quality outcomes.


Scope of Responsibilities:

The Clinical Nurse Specialist provides nursing care to individuals and groups requiring specialized knowledge, judgment, and skills derived from biological, physical, behavioral, social, spiritual/cultural, and nursing sciences.

This role operates within the full scope of nursing practice as defined by the Board of Nursing in their designated state, ensuring compliance with all hospital accrediting bodies.


Key Responsibilities:

  • Patient & Family Engagement

a. Recognize the patient or their designee as a key partner in delivering compassionate and coordinated care, respecting their preferences, values, and needs.

b. Gather and communicate patient values, preferences, and expressed needs.

c. Deliver patient-centered care with sensitivity and respect for diverse experiences, collaborating with the interdisciplinary team to engage patients or designated surrogates in active partnerships that promote health, safety, and well-being.

d. Address patient and family education needs, considering their comprehension levels and learning styles from entry through discharge.

e. Collaborate with patients, families, and the healthcare team regarding care provided and needed to ensure appropriate transitions along the continuum.

f. Evaluate patient responses and intervene to ensure optimal outcomes.


  • Evidence-Based Practice

a. Integrate the best current evidence with clinical expertise and patient/family preferences for optimal healthcare delivery.

b. Develop individualized care plans based on patient values, clinical expertise, and evidence.


  • Nursing Practice

a. Assess, formulate nursing diagnoses, plan, implement, and continually evaluate the effectiveness of individualized nursing care plans for each assigned patient.

b. Analyze and prioritize issues, making sound judgments based on logic and critical thinking.


  • Safety and Quality Improvement

a. Enhance the safety and quality of healthcare systems by minimizing risks to patients and the healthcare team.

b. Administer medications and execute regimens as authorized by state licensure.

c. Incorporate quality measures and data to assess individual and system performance.

d. Utilize technology and standardized practices that support safety and quality.

e. Identify strategies to mitigate risks to self or others.

f. Participate in unit-based quality improvement initiatives, evidence-based practice projects, or research.

g. Promote and implement error prevention techniques.

h. Report safety concerns and events, including near misses.


  • Informatics & Technology

a. Advocate for and incorporate technology to manage information, mitigate errors, and support decision-making.

b. Navigate, plan patient care, and document ongoing status, interventions, patient responses, and outcomes accurately and timely in the electronic health record.

c. Respond appropriately to clinical decision-making supports and alerts.

d. Safeguard the integrity of healthcare records and the privacy of each patient.


  • Communication, Teamwork & Collaboration

a. Engage effectively with patients, families, and the healthcare team to foster open communication, mutual respect, and shared decision-making.

b. Act as the coordinator of the patient's care plan through interaction and communication with the healthcare team.

c. Demonstrate integrity, consistency, and respect for diversity.

d. Operate within the scope of practice.

e. Delegate tasks to other team members as appropriate.

f. Address barriers, safety, and clinical concerns through the appropriate chain of command.


  • Leadership & Professionalism

a. Influence the behavior of individuals or groups to facilitate goal establishment and achievement, ensuring the delivery of nursing care aligns with ProMedica's core values, mission, philosophy, and policies.

b. Maintain a current understanding of the scope of practice as outlined by the state Board of Nursing.

d. Collaborate with healthcare teams to enhance patient care, work environment, and satisfaction for both patients and staff.

e. Take accountability for nursing practice by adhering to the nursing process.

Assign, direct, delegate, and supervise ancillary personnel and support staff in achieving patient care goals.

b. Organize and deliver patient care within the context of the work unit.

c. Participate in rounds with providers.

d. Implement bedside report/hand-offs.

e. Ensure meaningful hourly rounding occurs.

Qualifications:

Education:
The RN in Ohio must complete twenty-four contact hours of continuing education during their licensure period.

At least 1 hour of contact hours must relate to Chapter 4723 of the Ohio nurse practice code and rules.


Skills:
Adheres to the nursing process

Ability to respond effectively and efficiently to critical situations involving patients, personnel, and visitors in a professional manner

Competently performs basic nursing skills

Ability to evaluate daily practice for errors and pursue corrective actions

License:
Current State license as a Registered Nurse

Certification:
Current CPR training. Specialty certifications as required

WORKING CONDITIONS

Personal Protective Equipment:
Demonstrates knowledge and use of standard precautions and PPE. Knowledgeable in safe patient handling equipment.

Physical Demands:

Must tolerate exposure to dust, fumes, chemicals, temperature changes, and bloodborne pathogens and bodily fluids.

Must meet all health requirements as identified by the organization (e.g., PPD, respirator training, vaccines, as appropriate).

Must be able to move about the hospital and between workstations, and endure prolonged periods of standing. Must frequently move, lift, or carry light to heavy patients or equipment.

Must be capable of lifting medium materials, non-material handling; continuous standing, walking, bending, stooping, and reaching; frequent pushing, pulling, and squatting.

Occasional sitting, stairs, and kneeling.
