Job Summary

Cayuga Centers is seeking a skilled Behavioral Case Planner to join our team in Pittsburgh, PA. As a key member of our youth services team, you will provide case management and intensive behavior management services for Treatment Family Foster Care Youth and Families.

Key Responsibilities:

  • Develop and implement individualized behavior modification plans for youth
  • Provide oversight and support to Treatment Foster Families
  • Coordinate service provision and communicate with youth's family/guardian and placing agency
  • Act as a member of the youth's Treatment Team to ensure timely permanency

Requirements:

  • Bachelor's Degree in Human Services or related field and 2+ years of experience in intensive in-home service model or residential care
  • Master's degree in Human Services or related field with 1+ year of experience in foster care or intensive in-home service model
  • Experience in case planning and providing direct services to youth and families
  • Ability to support and provide feedback to foster parents
  • Valid PA Driver's License and safe, dependable vehicle
