We are seeking an experienced Care Coordinator to join our team in Sacramento.
Key Responsibilities- Assume responsibility for the operations of our organization and ensure the delivery of high-quality support services.
- Under the supervision of the Program Director, coordinate services for individuals with developmental disabilities.
- Develop and implement individualized support plans to meet the unique needs of our clients.
- Collaborate with internal and external stakeholders to ensure seamless service delivery.
- Provide supervision and guidance to support staff to ensure they are equipped to provide excellent care.
- Bachelor's degree in Psychology or a related field (Master's degree preferred) or 4 years of experience in the field of developmental disabilities.
- 2+ years of direct support experience with individuals with behavior challenges, medical involvement, or psychiatric diagnosis, and forensic involvement.
- 1+ year of experience coordinating services for individuals with developmental disabilities.
- 1+ year of experience in a supervisory position (preferred).
- Pay rate: $23-$25 per hour.
