Aveanna Healthcare Private Duty Nurse LVN/LPN

Job Summary:
Aveanna Healthcare is seeking a compassionate and skilled Private Duty Nurse LVN/LPN to join our team. As a Private Duty Nurse, you will provide one-on-one care to our patients in their homes, assisting them with transitioning from the hospital and providing continuous care to remain at home with their loved ones.
Responsibilities:
- Implement physicians' prescribed plans of care
- Conduct patient assessments and coordinate care
- Administer prescribed medication, treatments, and therapies
- Document care provided in a timely manner
- Provide health promotion, teaching, and training to patients and family members
- Accept and write verbal orders from physicians as required
- Follow up with physicians, clients, family members, staff, and Case Manager on client problems, concerns, and/or changes in client condition
Requirements:
- Active, unencumbered license as a Licensed Nurse in the state of practice
- Current CPR certification
- TB skin test (current within last 12 months)
- Six months prior hands-on nursing experience preferred but not required
- Reliable transportation
