Aveanna Healthcare Private Duty Nurse LVN

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN/LVN) to join our team at Aveanna Healthcare. As a Private Duty Nurse, you will provide one-on-one care to pediatric and adult patients in their homes, assisting them with transitioning from the hospital and providing continuous care to remain at home with their loved ones.
Responsibilities:- Implement physicians' prescribed plans of care for pediatric and adult patients
- Conduct patient assessments and coordinate care with the healthcare team
- Administer prescribed medication, treatments, and therapies as ordered by physicians
- Document all care provided in a timely and accurate manner
- Provide education and training to patients and their families on disease management, self-care techniques, and prevention strategies
- Collaborate with the healthcare team to ensure seamless care transitions and coordination
- Current, unrestricted state license as a Licensed Nurse in the state of practice
- Current CPR certification from American Red Cross, American Heart Association, or BLS
- Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures
- One year of experience working as a Licensed Nurse in a clinical care setting (requirements may vary by state)
