Aveanna Healthcare is a leading national provider of home health services to medically fragile children and adults. We are seeking a skilled and compassionate Private Duty Nurse LPN/LVN to join our team.
Job SummaryThe Private Duty Nurse LPN/LVN will provide one-on-one care to patients in their homes, assisting with daily living activities and providing medical care as needed. The ideal candidate will have a strong background in nursing, excellent communication skills, and a passion for delivering high-quality patient care.
Responsibilities- Implement and update the nursing care plan in collaboration with the patient, family, and healthcare team.
- Provide skilled nursing care to patients in their homes, including administering medications, wound care, and other medical treatments.
- Conduct patient assessments and coordinate care with the healthcare team.
- Document all patient interactions, assessments, and care provided in a timely and accurate manner.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Participate in ongoing education and training to stay current on best practices and industry developments.
- Current, unrestricted LPN/LVN license in the state of practice.
- Current CPR certification.
- One year of experience as a LPN/LVN in a clinical care setting.
- Ability to lift 50 pounds and stand for long periods.
- Strong communication and interpersonal skills.
