The Case Manager RN ensures high-quality, patient-centered care by managing rehabilitative care to ensure optimum outcomes. This role provides, coordinates, and directs care specific to the needs of each Rehab patient.
Key Responsibilities- Provide program orientation to patients/families/caregivers, including the case manager's role, Rehab philosophy, and continued stay and discharge criteria.
- Act as the coordinator of patient/family/caregiver education, promoting participation in team discussions related to plans, goals, and status.
- Ensure the implementation of the patient's treatment plan that supports the patient's strengths, abilities, needs, and preferences.
- Document the findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments, communicating social, financial, or discharge needs and preferences of the patient/family/caregiver.
- Assume accountability for promoting consistent, positive patient interactions that advance the agenda of unparalleled patient service.
- Minimum of 3 years in nursing practice, with clinical knowledge, IT skills, effective interpersonal communication, critical thinking, successful problem-solving, ability to work autonomously, innovative thinking, and resourcefulness in navigating community resources.
- Preferred experience: Previous 5 years in nursing practice and 3 years care management, community or ambulatory care management experience, primary care clinical experience, experience with inpatient discharge planning work, Motivational Interviewing, and Intrinsic Health Coaching.
- Required Education: Bachelor of Science in Nursing or bachelor's degree in a related field. If RN, effective 3/1/18 Bachelor of Science in Nursing OR enroll in a BSN or advanced degree program within 6 months AND obtain a BSN within 4 years of hire date into RN position.
- Preferred Education: Master's in Nursing.
- Required License: If RN, must have and maintain current licensure as registered nurse with the North Carolina Board of Nursing.
- Required Certification: BLS Healthcare Provider.
- Preferred Certification: Level I: Maintains nationally recognized certification in Case Management, such as ACM, CCM or ANA certification.
