Job Summary

We are seeking a highly motivated and experienced Bilingual Case Manager to join our team. The successful candidate will be responsible for providing case management services to children and families, including conducting assessments, developing service plans, and coordinating with community resources.

Key Responsibilities:
  • Conduct assessments and develop service plans for children and families
  • Coordinate with community resources to provide support and services
  • Monitor and evaluate the effectiveness of service plans
  • Provide crisis intervention and support as needed
Requirements:
  • Bachelor's degree in Social Work, Counseling, or related field
  • 1 year of experience in case management or a related field
  • Bilingual (English and Spanish) language skills
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
