Position Overview


The Clinical Nurse Specialist collaborates within a multidisciplinary team to deliver primary and specialized care to patients facing various chronic health conditions.

This role involves managing treatment adherence challenges and unstable health statuses related to chronic disease management, focusing on medical compliance, self-management, health promotion, and harm reduction strategies.

The nurse will develop comprehensive care plans, connect patients with essential internal and external resources (such as mental health services, substance abuse support, and specialist referrals), and monitor medical progress in alignment with premier quality care standards.

Role-Specific Responsibilities:

  • Possess knowledge of HIV/AIDS-related conditions and substance abuse, including current treatment and adherence strategies, as well as risk reduction intervention techniques.
  • Understand chronic health issues such as diabetes, hypertension, and heart disease, with an awareness of the needs of individuals and families throughout the primary care continuum.
  • Assess patients' needs concerning treatment education, risk reduction, or preventive measures.
  • Exhibit strong communication skills, effectively collaborating within a multidisciplinary team to provide patient care.
  • Conduct sensitive and empathetic interviews that honor the dignity and diversity of patients.
  • Prepare accurate, concise, and comprehensive documentation.
  • Maintain patient records and information in a precise, timely, and confidential manner.

Essential Knowledge, Skills, and Abilities:

  • Ability to work independently with minimal oversight.
  • Effective time management skills to achieve strategic objectives.
  • Understanding of principles and processes for delivering excellent customer service.
  • Familiarity with all applicable Whitman-Walker policies and procedures.
  • Strong oral and written communication abilities.
  • Knowledge of healthcare information systems, with adaptability to new electronic medical records and population health platforms.
  • Analytical skills for preparing accurate and concise reports.
  • Ability to maintain records and information in an accurate, timely, and confidential manner.
  • Flexibility to adapt to changes in service delivery models and re-prioritize tasks as necessary.
  • Willingness to work various shifts, including evenings and weekends as required.
  • Commitment to health equity.

Preferred Qualifications:

  • Fluency in medical terminology.
  • Familiarity with eCW EMR (preferred but not required).

Education and Experience:

  • Bachelor of Science in Nursing or equivalent work experience required.
  • Master of Science in Nursing or Public Health preferred.
  • 1-3 years of experience post RN licensure preferred.
  • Active Registered Nurse licensure in the District of Columbia.
  • Experience working with a diverse workforce preferred.
  • Ability to collaborate effectively with diverse groups required.
  • 1 year of experience with individuals living with HIV or related care preferred.

Working Conditions:
Standard office environment. Flexibility to work evenings, weekends, and remotely as needed.

Physical Demands:
  • Lifting: Up to 20 lbs. infrequently.
  • Movement: Prolonged standing and sitting.
  • Visual: Extended periods on a computer.
  • Concentration: High focus required for data entry and telephone communication.
  • Communication: Competency in both written and verbal communication.
