Our specialists provide support to patients at home between medical appointments through remote patient monitoring (RPM) and chronic care management (CCM), aiming for improved health outcomes.

In November 2021, our COVID-19 Antigen Rapid Test received Emergency Use Authorization from the U.S. Food and Drug Administration for over-the-counter distribution. Since then, we have become a vital supplier of at-home COVID tests to various governmental and non-governmental entities.

Desired Attributes:
  • Technologically Proficient: Ability to explain mobile application technology to a non-technical, elderly audience and troubleshoot mobile device and application issues.
  • Familiarity with G-Suite: Comfortable utilizing internal communication tools (Google Hangout, Zoom, Google Sheets, PDFs, printers, etc.).
  • Empathetic and Patient: Our primary patient demographic is elderly, requiring sensitivity, adaptability in teaching methods, and patience.
  • Growth-Oriented Mindset: As our organization evolves, so do our protocols and workflows. We seek team members who can embrace feedback and contribute to continuous improvement.
  • Bilingual Skills: Proficiency in languages such as Spanish, Cantonese, Mandarin, Tagalog, or Vietnamese is advantageous.

Licenses/Certifications Required:
Registered Dietitian/Nutritionist (RD/RDN)

Key Responsibilities:

  • Assist in educating elderly patients on the functionality of our mobile application and medical devices, while addressing any technical challenges.
  • Collaboratively create personalized care plans, behavioral goals, and educational objectives for each patient, ensuring HIPAA compliance in documentation.
  • Apply established Standards of Practice and Professional Performance (SOP/SOPP) in diabetes care and evidence-based guidelines to tailor educational interventions.
  • Work in conjunction with primary care physicians and other healthcare professionals.
  • Utilize iHealth Unified Care software and patient vitals from our devices to monitor outcomes and facilitate routine goal setting and educational support.
  • Provide assistance to patients through our online platform during scheduled shifts and visits.
  • Collaborate with a cross-functional product team to enhance digital health solutions, extending chronic disease management beyond traditional healthcare settings.
  • Potential opportunities to mentor colleagues within our clinical service team, contribute to clinical protocol enhancements, or engage in internal research initiatives.
  • Depending on expertise and interest, create educational content such as blog articles, videos, and handouts for our chronic disease self-management education program, adhering to AADE and AHA standards.
  • Other responsibilities and projects as assigned.
