Role Scope:

We are seeking a motivated and compassionate dietitian who is eager to contribute to the well-being of elderly individuals. The Dietitian will collaborate with a multidisciplinary care team, led by a primary care provider, to deliver integrated and holistic care. Responsibilities include conducting nutritional assessments, planning and implementing tailored nutrition interventions, and providing education to participants and their caregivers.

Key Responsibilities:

  • Develop and execute personalized nutrition care plans that consider medical needs and social circumstances in collaboration with the primary care provider and interdisciplinary team.
  • Conduct follow-ups and reassessments for participants, adjusting plans as necessary based on changes in their health status.
  • Evaluate participants' access to food resources and coordinate referrals for meal preparation and grocery shopping assistance.
  • Assess the need for liquid nutritional supplements and manage orders accordingly.
  • Provide guidance to participants and caregivers on dietary regimens and nutritional needs, including home visits when required.
  • Implement nutrition care plans for high-risk participants, incorporating medical nutrition therapy recommendations.
  • Monitor dietary intake and therapeutic diets, communicating relevant information and concerns with the care team.
Administrative Duties:
  • Perform sanitation and safety checks for kitchen facilities and equipment as needed.
  • Review and adjust lunch and snack menus based on individual dietary requirements.
  • Maintain updated records for dietary plans and assist in dining room operations.
  • Participate in care planning sessions, providing insights on participants' nutritional statuses.
  • Collaborate with state food programs in compliance with regulations.
  • Act as a subject matter expert in developing standardized dietary programs and conducting team training.
Qualifications:
  • Bachelor's degree in nutrition, dietetics, or a related field.
  • Certification as a registered dietitian by the Commission on Dietetic Registration.
  • A minimum of one year of experience working with elderly populations.
  • Proficiency in medical terminology and documentation of participant care.
  • Current medical clearance for communicable diseases and up-to-date immunizations.
Preferred Qualifications:
  • Two or more years of experience as a practicing dietitian.
  • Bilingual abilities in Spanish, Mandarin, or Cantonese are advantageous.
  • Possession of a valid driver's license and personal transportation.
