Role Scope:

We are seeking a proactive and compassionate Dietitian who is committed to enhancing the well-being of elderly individuals. This role involves working within a multidisciplinary team, led by a primary care provider, to deliver integrated, whole-person care. The Dietitian will conduct nutritional assessments, monitor participants' dietary needs, and implement tailored nutrition interventions. Additionally, this position will oversee the development of our medical nutrition program for participants.

Participant Care Responsibilities:

  • Formulate and execute personalized nutritional care plans that consider participants' social and medical circumstances, in collaboration with the primary care provider and interdisciplinary team.
  • Conduct follow-ups and reassessments for each participant, adjusting plans as necessary based on changes in their health status.
  • Evaluate participants' access to food resources and refer them to the interdisciplinary team for assistance with meal preparation and grocery shopping.
  • Identify the need for liquid nutritional supplements and order them accordingly.
  • Provide guidance to participants and caregivers regarding dietary regimens and nutritional needs, which may include home visits.
  • Develop and implement nutrition care plans for high-risk participants, including recommendations for medical nutrition therapy.
  • Monitor dietary intake, therapeutic diets, and any necessary dietary modifications, communicating relevant information to the care team.
Administrative and Program Development Duties:
  • Perform sanitation and safety inspections of kitchen facilities and equipment as required.
  • Assess the adequacy of meal menus and make necessary adjustments based on individual dietary needs.
  • Update dietary documentation, including lunch lists and dietary guides, as needed.
  • Participate in care planning meetings, providing updates on participants' nutritional statuses.
  • Collaborate with state food programs in accordance with regulatory guidelines.
  • Serve as a subject matter expert in developing standardized dietary programs and conducting team training.
Qualifications:
  • Bachelor's degree in nutrition, dietetics, or food science.
  • Proficiency in medical terminology and documentation of participant care.
  • Registered Dietitian certification from the Commission on Dietetic Registration.
  • A minimum of one year of experience working with elderly or frail populations.
  • Verification of medical clearance for communicable diseases, including a TB test.
  • Current immunizations must be up to date.
Preferred Qualifications:
  • At least two years of experience as a practicing dietitian.
  • Bilingual skills in Spanish, Mandarin, or Cantonese are preferred.
  • A valid driver's license and personal transportation as required by law.
Compensation:

Compensation will be determined based on qualifications, skills, and experience. This position includes eligibility for medical, dental, and vision insurance, as well as other benefits such as 401(k) savings plans and paid time off.
