Job Summary:

The Medical Social Worker II is a critical member of the healthcare team responsible for implementing the plan of care for patients. This position requires collaboration with patients, families, and interdisciplinary care team members to ensure safe, timely, and efficient care.

The ideal candidate will have a Master's degree in Social Work from a Council on Social Work Education-accredited program and licensure as an LCSW-C. This role involves:

  • Developing and implementing patient care plans
  • Collaborating with healthcare team members to ensure continuity of care
  • Identifying and facilitating community resources
  • Advocating for patient and family needs
  • Providing education and navigation to patients as needed
  • Educating hospital staff on patient psychosocial issues

This position requires strong problem-solving skills, innovative thinking, and effective communication. The successful candidate will be a team player who can work collaboratively with healthcare professionals to achieve exceptional patient outcomes.

Requirements:
