Job Summary

The Medical Social Worker II is a key member of the healthcare team, responsible for implementing the plan of care for patients and families. This role requires strong communication and collaboration skills, as well as the ability to work effectively with interdisciplinary teams to ensure safe, timely, and efficient care.

Key Responsibilities
  1. Assessment and Planning: Conduct thorough assessments of patients and families to identify their needs and develop individualized plans of care.
  2. Collaboration and Communication: Work closely with healthcare teams to ensure seamless communication and coordination of care.
  3. Patient and Family Support: Provide emotional support and education to patients and families, helping them navigate the healthcare system and make informed decisions about their care.
  4. Community Resource Referral: Identify and connect patients and families with community resources and services to support their needs.
  5. Advocacy and Problem-Solving: Advocate for patients and families, addressing any concerns or issues that may arise during their care.
Requirements
  • Master's degree in social work from a Council on Social Work Education-accredited program.
  • Licensure as an LCSW-C is preferred, but LMSW may be considered with appropriate work history.
