This is a full-time position that requires a high school diploma or equivalent, as well as two years of paid experience performing various functions, including general outreach, engagement, and follow-up with members. The primary responsibilities of the Extender include coordinating services and appointments, engaging clients in health promotion activities, sharing information with the Care Manager, making referrals, participating in case conferences, and supporting the care manager in assessing and addressing unmet health-related resource needs.
Key Responsibilities:
- Outreach and Engagement: Perform general outreach, engagement, and follow-up with members to ensure they receive necessary support.
- Service Coordination: Coordinate services and appointments to meet the unique needs of each client.
- Health Promotion: Engage clients in health promotion activities to improve overall well-being.
- Communication: Share information with the Care Manager on the member's circumstances to ensure comprehensive support.
- Referrals: Make and track referrals to ensure clients receive necessary resources.
- Care Planning: Participate in case conferences to develop effective care plans.
- Support: Support the care manager in assessing and addressing unmet health-related resource needs.
Requirements:
- Education: High school diploma or equivalent required.
- Experience: Two years of paid experience performing various functions, including general outreach, engagement, and follow-up with members.
- Skills: Strong communication and interpersonal skills, ability to work with diverse populations, and proficiency in service coordination.
Estimated Salary Range: $45,000 - $55,000 per year, depending on location and experience.
