Job Summary:

This full-time direct care position requires a high school diploma or equivalent and two years of paid experience working with behavioral/mental health clients.

The primary responsibilities include general outreach, engagement, and follow-up with members, coordinating services/appointments, engaging clients in health promotion activities, sharing information with the Care Manager, making and tracking referrals, participating in case conferences, and supporting the care manager in assessing and addressing unmet health-related resource needs.

Estimated Salary: $45,000 - $60,000 per year based on qualifications and experience.
