Position Overview

Title: Human Services Community Coordinator

Reports To: Human Services Program Coordinator

Work Schedule: Full Time, Monday to Friday, 8:00 AM - 4:30 PM; Non-Exempt

Key Responsibilities

  • Oversees groups of participants in various environments, including work placements, volunteer opportunities, recreational activities, and community events.
  • Creates and sustains a range of volunteer, vocational, and educational opportunities for individuals.
  • Contributes to the design of innovative and engaging curricula to achieve program objectives.
  • Incorporates independent living skills, employment readiness, self-advocacy, and interpersonal skills into daily programming.
  • Facilitates transportation for individuals to and from community engagements and employment sites.
  • Assists in the preparation of necessary assessments and documentation, including incident reports and progress summaries.
  • Engages in ongoing training and professional development initiatives.
  • Addresses any unsafe interactions or behavioral incidents to ensure participant safety.
  • Participates in Individual Support Plan (ISP) meetings with relevant stakeholders as required.
  • Collaborates with the Program Coordinator to gather accurate data for ISPs, behavior plans, and monthly progress reports.
  • Works alongside other Community Coordinators and the Program Coordinator to ensure high-quality service delivery.
  • Regularly communicates progress and challenges to the Program Coordinator.
  • Represents Triangle positively to clients, families, and community organizations.
  • Participates in biweekly team meetings and monthly supervision sessions with the Program Coordinator.
  • Undertakes additional responsibilities as assigned.

Qualifications

  • High school diploma or equivalent.
  • Some college coursework in a related field and/or one to three years of relevant experience preferred.
  • Valid driver's license and a clean driving record.
  • Successful completion of State and Federal Background Checks.

Language Proficiency

Ability to effectively communicate verbally, read, analyze, interpret, and compose documents. Capable of addressing common inquiries or concerns from regulatory bodies or family members.

Physical and Work Environment Requirements

The physical demands of this role include occasional standing, walking, sitting, and the ability to lift or move up to 25 pounds. Reasonable accommodations may be made for individuals with disabilities. The work environment is typically moderate in noise level.
