Position Summary:
As an RN Care Coordinator, you will play a vital role in our Integrated Care Model by effectively communicating member needs and advocating for optimal care solutions. You will ensure that members receive the appropriate services tailored to their individual requirements. Our team comprises RNs, LVNs, and CACs who are committed to assisting members in achieving their health objectives. Collaboration with nursing facilities where our members reside is essential.

Key Responsibilities:
  • Evaluate, strategize, and execute personalized care plans aimed at providing the most suitable and least restrictive care options.
  • Identify and facilitate referrals for members transitioning to community services, and assist with social service programs, including financial, psychosocial, and state-supported services.
  • Oversee the care plan throughout the entire care continuum, serving as a consistent point of contact.
  • Engage with all relevant parties to ensure timely and quality coordinated care and services for all members.
  • Advocate for members and their families to ensure their needs and preferences are fully acknowledged and supported by the health care team.

Travel Requirements:
This position requires local travel approximately 50% of the time based on business needs.

Qualifications:
Required:
  • Current, unrestricted RN license in Texas.
  • A minimum of 2 years of experience in a community health setting or a related health care role.
  • Proficiency in Microsoft Office Suite, including Word, Excel, and Outlook.
  • Reliable transportation for local travel to visit members in various settings.
  • Access to high-speed internet at home.

Preferred:
  • At least 1 year of experience with long-term care services and support, Medicaid, or Medicare.
  • Ability to create, edit, save, and send documents, spreadsheets, and emails.
  • Understanding of integrated care settings and relevant federal and state regulations.
  • Reside within a reasonable commuting distance of assigned duties.
