The Nurse Care Coordinator is responsible for planning, organizing, and directing home care services. This role requires a Registered Nurse with experience in community health education and a strong emphasis on building relationships with individuals and families in their homes and communities.
Key Responsibilities:
- Plan and direct home care services to meet the needs of individuals and families
- Build relationships with community resources to support patient care
- Collaborate with healthcare teams to ensure seamless care transitions
- Provide education and support to patients and families on health management and self-care
Requirements:
- Current and unencumbered RN license in the state of employment
- Bachelor's degree in Nursing from an accredited program
- One to two years of recent acute care experience in an institutional setting
- One year of home health care experience preferred
- Management experience not required, but direct supervisory experience of LVNs and NAs preferred
