Job Summary

The Nurse Care Coordinator is responsible for planning, organizing, and directing home care services. This role requires a Registered Nurse with experience in community health education and a strong emphasis on building relationships with individuals and families in their homes and communities.

Key Responsibilities:

  • Plan and direct home care services to meet the needs of individuals and families
  • Build relationships with community resources to support patient care
  • Collaborate with healthcare teams to ensure seamless care transitions
  • Provide education and support to patients and families on health management and self-care

Requirements:

  • Current and unencumbered RN license in the state of employment
  • Bachelor's degree in Nursing from an accredited program
  • One to two years of recent acute care experience in an institutional setting
  • One year of home health care experience preferred
  • Management experience not required, but direct supervisory experience of LVNs and NAs preferred
