About the Role

We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team in Cleveland, OK. As a member of our interdisciplinary team, you will provide high-quality, patient-centered care to our complex elderly population in the comfort of their own homes.

Key Responsibilities
  • Manage chronic illnesses and provide comprehensive care to patients with multimorbid conditions
  • Document patient information and close quality gap measures
  • Conduct initial assessments, including functional evaluations, social and safety assessments, cognitive screening, and mental health assessments
  • Triage, diagnose, and treat patients on an urgent basis in their homes
  • Order and interpret tests, including x-rays, labs, and EKGs, to diagnose and treat patients
  • Collaborate with patient's PCP and specialists as needed

Requirements

To be considered for this role, you must have:

  • Graduate from an accredited NP or PA program and have Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or National Commission on Certification of Physician Assistants (NCCPA)
  • Active and unrestricted State Nurse Practitioner license or Physician Assistant (PA-C) in the state where the job is located
  • Valid DEA license or the ability to obtain one prior to employment
  • Access to reliable transportation
