About the Job

As a Cultural Navigator Specialist at One Community Health, you will work collaboratively with all members of the care team and extended medical team to deliver patient education in the form of printed materials, group health workshops, telephonically, and verbally during visits to the health center. This includes assisting with outreach telephone calls to reduce care gaps and keep patients engaged in care, remaining current on health-related community resources, and acting as a point of contact for patients needing assistance with accessing services. You will also assist with translation of patient-facing signage, documents, and educational material in collaboration with the communications department, and represent OCH at community events, providing information about services in multiple languages.

A degree or certification in healthcare administration, interpretation, or linguistics is preferred, but not required. The ideal candidate will have excellent interpersonal and communication skills, the ability to problem solve independently, and the ability to collaborate in a team setting. We offer a competitive salary range of $70,000 - $80,000 per year, depending on experience, plus a comprehensive benefits package.
