Job Description

Freedom Home Care seeks a skilled RN Field Supervisor to oversee the delivery of high-quality care to our clients. This role involves coordinating with colleagues to assess, plan, implement, or evaluate patient care plans, ensuring that each client receives tailored support.

Key Responsibilities

  1. Collaborate with healthcare professionals to develop comprehensive care plans for each client.
  2. Conduct regular home visits to monitor client progress and make adjustments to care plans as needed.
  3. Communicate effectively with clients and their families, addressing concerns and providing guidance on care management.
  4. Recruit, train, and supervise caregivers, ensuring they possess the necessary skills to deliver exceptional care.
  5. Maintain accurate records of client interactions, including progress notes and care plan updates.
  6. Participate in on-call rotations to address emergency situations and provide timely support.

Qualifications

  • Bachelor's degree in nursing or a related field; or 2 years' experience in geriatrics and/or care management.
  • Current licensure as a Registered Nurse (RN) in the state of employment.
  • Prior experience as a Registered Nurse is preferred.
  • Excellent interpersonal and leadership skills.
  • Valid driver's license.
  • A strong commitment to delivering exceptional client care.

Salary

$75,000 - $95,000 per year, based on experience and location.
