Mental Health Case Manager Position in South Central Minnesota

Join our team as a Childrens Mental Health Case Manager in Steele County and make a meaningful difference in the lives of children and families. As a key member of our staff, you will work collaboratively with clients, families, and community providers to deliver comprehensive mental health services.
Key Responsibilities- Deliver services according to SCHRC and State of Minnesota guidelines for Childrens Mental Health Case Management.
- Maintain accurate and up-to-date records, including individual client records, time records, and travel records.
- Develop and implement Individual Treatment Plans (ITPs) based on client needs and goals.
- Collaborate with clinical supervisors, case managers, and other stakeholders to ensure effective service delivery.
- Bachelor's degree in one of the behavioral sciences or related fields, including but not limited to social work, psychology, or nursing.
- At least two years of experience in human services, preferably in childrens mental health case management.
- Valid driver's license, reliable vehicle, and auto insurance.
- A competitive salary range of $45,000-$60,000 per year, commensurate with experience.
