Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our psychiatric unit team at Foundations for Living. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the overall success of our team.

Key Responsibilities
  • Deliver nursing care consistent with the treatment plan, working under the supervision of the Staff Nurse or Charge Nurse.
  • Conduct thorough assessments of patients, documenting their medical and physical condition accurately.
  • Attend to patients' personal needs, including oral hygiene, toileting, bathing, dressing, and ambulating as required.
  • Assist with vital sign monitoring, including pulse, blood pressure, temperature, and respiration.
  • Collaborate with the Medical Director and registered nurses to ensure seamless patient care.
  • Administer medications and injections as needed, adhering to approved methods and protocols.
  • Participate in seclusion or restraint procedures, observing safety precautions and physical requirements.
  • Interpret nursing and treatment needs to non-clinical staff, promoting effective communication and teamwork.
  • Maintain patient dignity and respect patient values, creating a compassionate environment.
  • Ensure patient privacy and confidentiality, adhering to ethical standards and regulatory requirements.
  • Collaborate with physicians, multidisciplinary teams, and outside health agencies to provide efficient discharge plans and accurate patient histories.
  • Document all required information in a timely and efficient manner, including admission paperwork, assessments, progress notes, interventions, evaluations, and discharge paperwork.
Requirements
  • Successful completion of a state-approved training program.
  • State LPN certification/license.
  • CPR certification (if required by state).
  • Minimum one year of experience preferred, with appropriate education.
