Job Title: Health Promoter

UNITE HERE HEALTH is seeking a skilled Health Promoter to join our team. As a key member of our outreach team, you will play a vital role in promoting health and wellness among our participants.

Job Summary:

The Health Promoter will work collaboratively with our nurses and health center staff to facilitate communication and coordinate care to improve health outcomes of our participants. This role requires a strong understanding of healthcare, service sector labor unions, and community organizing.

Key Responsibilities:

  • Meet with participants to educate and motivate them regarding participation in Fund programs/care coordination designed to improve health outcomes
  • Recruit participants into Fund self-management programs and follow up with them to ensure maximum participation
  • Coordinate with contracted Case Management partners and community resources to ensure appropriate services are available
  • Meet with participants at their worksite, home, union office, and places where they receive medical services
  • Review data to determine priority targets for program outreach
  • Arrange, coordinate, and facilitate trainings and group programs, participant leader meetings, and other health education events
  • Work with participant leaders to expand participation in these programs
  • Document work and outcomes in CarMa
  • Provide input to management regarding concerns identified through interactions with participants

Requirements:

  • 2-3 years of related experience minimum
  • Spanish bi-lingual
  • Working knowledge and experience in healthcare, service sector labor unions, healthcare community organizing, healthcare customer service, and/or health outreach and education
  • Associate degree or equivalent work experience required
