Patient Care Specialist

The Patient Care Technician (PCT) is a vital member of the multi-disciplinary treatment team at Community Bridges, Inc. (CBI), providing direct care to behavioral health patients under the medical direction of a Registered Nurse (RN).
The primary focus is on delivering high-quality, patient-centered care as directed and delegated by the RN, Milieu Lead, Operation Supervisor, Operations Administrator, or Program Manager.
The PCT is also responsible for ensuring the safety of the unit they are assigned to, identifying environmental and patient-specific safety concerns, and elevating those concerns to the RN, RN Supervisor, Operation Supervisor, or designee as needed.
Key Responsibilities- Provide direct patient care as directed by the RN and other healthcare professionals
- Assist with medication administration, vital signs, and other medical procedures
- Conduct patient assessments and document observations
- Participate in treatment planning and implementation
- Collaborate with the treatment team to ensure seamless care delivery
- Identify and report safety concerns to the appropriate personnel
- High school diploma or equivalent required; Associate's degree or higher preferred
- Minimum 6 months of healthcare-related experience with exposure to behavioral and medical crisis situations
- Ability to work in a fast-paced, dynamic environment
- Strong communication and interpersonal skills
- Ability to maintain confidentiality and handle sensitive information
