Service Coordinator for Aging Communities

The Service Coordinator plays a vital role in supporting residents at Presbyterian Villages of Michigan, ensuring they maintain a good quality of life and age in place.
Key Responsibilities- Network with community organizations to identify affordable services for residents.
- Conduct psychosocial assessments to evaluate residents' current condition and identify areas for improvement.
- Provide case management and referral services, maintaining accurate and up-to-date resident files.
- Coordinate services to promote optimal living, including aging in place and wellness philosophies.
- Develop and implement care plans, monitoring and managing supportive services as needed.
- Submit regular performance reports to ensure continued funding for the Service Coordination program.
- Engage residents to identify areas of need, making necessary referrals and promoting resident involvement in on- and off-site programs.
- Assist in coordinating home care services, developing a resource directory of service providers.
- Provide counseling, health screenings, and legal advocacy as needed.
- Minimum degree in social work, psychology, or gerontology preferred.
- Two or more years of experience in social service delivery, with a focus on elderly and family populations.
- Working knowledge of supportive services and resources in the area served by the project.
- Ability to advocate, organize, problem-solve, and provide results for residents.
- Strong communication, comprehension, and interpersonal skills.
- Proficiency in Windows, Microsoft Office Suite, and resident software data systems (AASC online, Care Guide, RealPage, etc.).
