- Travel to participants' homes for site visits as needed.
- Participate in quality planning and have a hands-on opportunity to directly impact quality of care.
We are looking for a highly motivated and experienced Licensed Social Worker who is passionate about serving others. The ideal candidate will have at least 1 year of experience in an elderly patient setting, a bachelor's or master's degree from an accredited School of Social Work, a valid social work license, CPR certification, a valid driver's license, and valid auto insurance and reliable transportation.
