As a key member of our clinical team, you will play a vital role in delivering high-quality, patient-centered care to individuals with advanced illnesses.

Key Responsibilities
  • Collaborate with our interdisciplinary team to develop and implement comprehensive care plans for patients with advanced illnesses.
  • Provide diagnosis, treatment, and follow-up care to patients, working under the direction of our Palliative Medical Director or Primary Care physician.
  • Conduct thorough assessments of patients' physical, emotional, and social needs, identifying areas for improvement and developing strategies to address them.
  • Establish and maintain effective working relationships with patients, families, and other healthcare professionals to ensure seamless care coordination.
  • Prescribe medications, including controlled substances, as delegated and licensed, and order laboratory, imaging, and other diagnostic tests as needed.
  • Communicate clinical findings and treatment plans to patients, families, and healthcare professionals, and re-assess the effectiveness of care plans on a regular basis.
  • Participate in evening and weekend call as required, providing on-call services in a clinically competent and responsive manner.
Requirements
  • Master's degree in Nursing from an accredited school of nursing, or equivalent as allowed under state law.
  • Minimum of two years of experience as a Nurse Practitioner, with a specialty in Family or Adult Medicine preferred.
  • Value-Based Care practice experience and Part B billing experience are also preferred.
  • Current license as a registered nurse or equivalent as required by state law for Nurse Practitioner licensure.
  • Current Nurse Practitioner certification/license, and a collaborative agreement with a supervising physician that is part of the Professional Services Corporation.
  • Part B Provider certification, Federal DEA license, and State DEA license (if required).
