POSITION TITLE: HOME HEALTH AIDE

SUPERVISOR TITLE: Registered Nurse/Nurse Supervisor

Key Responsibilities:
  • Deliver personal care and related services in the client's residence, under the guidance and supervision of the nursing staff.
  • Perform tasks assigned by a Registered Nurse (RN), ensuring that all care provided is under the RN's oversight.
  • Adhere strictly to the care plan to ensure safe and competent service delivery.
  • Assist clients in maintaining personal hygiene and a safe living environment.
  • Prepare nutritious meals and assist with grocery shopping as directed by the nursing staff.
  • Support clients with mobility as prescribed by their healthcare provider and supervised by nursing personnel.
  • Collaborate with therapy staff to aid in rehabilitation processes as needed.
  • Encourage client independence in accordance with the nursing care plan.
  • Engage clients in activities that promote mental alertness and well-being.
  • Provide emotional and psychological support to clients and their families, fostering trust and confidentiality.
  • Report any changes in the client's condition or home environment to the supervising nurse.
  • Conduct routine housekeeping tasks to maintain a safe and comfortable environment for the client.
  • Complete visit reports promptly and incorporate them into the clinical record on a weekly basis.
  • Coordinate visit schedules to ensure efficient care delivery by all staff members.
  • Work collaboratively with community agency personnel involved in the client's care as directed by nursing staff.
  • Participate in required in-service training sessions.
  • Possess the ability to drive and access clients' homes, which may not be wheelchair accessible.
  • Demonstrate sufficient hearing, eyesight, and physical dexterity to perform client assessments and care.
  • Occasionally required to lift or move clients weighing up to 250 pounds and carry items up to 30 pounds.
  • Communicate effectively in English, both verbally and in writing.
  • Utilize basic medical equipment such as BP cuffs, thermometers, and stethoscopes, along with hand hygiene supplies.
  • Maintain access to client medical records for discussion with the supervising nurse.
Qualifications:
  • High school diploma or equivalent preferred.
  • Completion of a training and competency evaluation program as specified in relevant regulations.
  • Must be free from health issues that could be harmful to clients or coworkers.
  • Understanding of personal care, housekeeping, and meal preparation fundamentals.
  • Respect for client privacy, ethics, and confidentiality of care.
  • Current CPR certification required.
  • Successful completion of a criminal background check.
