Job Summary

We are seeking a highly motivated and compassionate HIV Health Navigator to join our team at CommonSpirit Health. As a key member of our Medical Care Coordination program, you will play a critical role in planning, coordinating, and implementing patient retention activities to re-engage patients living with HIV/AIDS back into medical care.

Key Responsibilities
  • Identify and Re-engage Patients: Work with the MCC team to locate and re-engage patients who may be out of healthcare completely or underutilize services.
  • Schedule Follow-up Appointments: Schedule follow-up appointments and conduct follow-up activities to ensure patients return for follow-up visits.
  • Coordinate Patient-Centered Activities: Assist in the coordination and delivery of patient-centered activities that focus on addressing health status, engagement, and retention in care, adherence to HIV medications, and HIV risk reduction.
  • Conduct Field Investigations: Work independently conducting office and field investigations (site visits to patients' homes, neighborhoods, shelters, etc.) to locate, assess, and refer individuals for necessary medical evaluation and treatment.
  • Track Engagement and Retention: Track engagement and retention in medical care.
  • Maintain Confidentiality: Maintain confidentiality of protected health information in compliance with state and federal law.
  • Participate in Meetings: Participate in staff meetings, Medical Care Coordination meetings, case conferencing, and other meetings as required.
  • Enter and Maintain Program Data: Enter and maintain program data, including clinical documentation in Electronic Health Records (EHR).
  • Prepare Reports: Enter monthly service data in Casewatch in a timely manner and prepare other demographic or statistical reports as required by the funding agency and Program Manager.
Qualifications
  • Experience in HIV Prevention or Care: Two years of experience conducting outreach to engage individuals in HIV prevention or care.
  • HIV Field Experience: Three years of working in the HIV field.
  • Supporting At-Risk Populations: Three years of supporting LGBTQ, Latino, Black, street-involved, behavioral illness, drug-using, and/or sex worker populations.
  • Knowledge of Insurance Eligibility: Knowledge of Medi-Cal, Medicare, Ryan White, and private insurance eligibility and enrollment structures.
  • HIV Risk Behaviors: Knowledge of HIV risk behaviors, substance abuse, STIs, and behavior change principles and strategies.
  • Community Resources: Familiarity with social service resources in the local community preferred.
  • Communication Skills: High level of compassion and ability to communicate clearly with at-risk populations.
  • Teamwork: Proven track record in a team-centered work environment focused on collaboration.
  • Ethical Delivery of Care: Ability to deliver care in an ethical, nonjudgmental, culturally competent, and nondiscriminatory manner.
  • Passion for Role: Highly motivated and driven passion to take ownership of their role within the team.
