PRIMARY DUTIES:

  1. Conduct laboratory tests based on standing or new orders and accurately document vital signs.
  2. Assess physical conditions for evaluation and treatment by healthcare providers.
  3. Ensure timely follow-up on lab results and prepare documentation for providers.
  4. Act as a liaison between clients and healthcare providers.
  5. Facilitate referrals to other medical professionals and facilities as needed.
  6. Communicate effectively with agencies, ancillary service providers, hospitals, and others as directed by the physician or nurse practitioner.
  7. Exemplify PATH Integrated Health's commitment to compassion and kindness in every client interaction.
  8. Work collaboratively and respectfully with clients dealing with substance use disorders, behavioral issues, and mental health challenges.
  9. Uphold ethical standards of practice as defined by licensing and professional clinical guidelines. Adhere to the Ethical Code of Ethics relevant to the profession.
  10. Demonstrate respect for cultural diversity, religious beliefs, and personal values of clients and their families, aligning with the company's mission.
  11. Maintain clear, concise, and timely documentation of client interactions.
  12. Assist clients with self-administration of medication in a supportive living environment.

REQUIREMENTS:

Licensure: Unrestricted State nursing license as an LPN.

Experience: A minimum of 2 years of nursing experience is preferred.

PHYSICAL DEMANDS: The role requires the ability to perform under pressure and to work flexible hours. It involves prolonged periods of sitting, viewing computer screens, and typing.

COMMUNICATION SKILLS: Candidates must possess excellent verbal and written communication abilities to interact professionally via phone and in writing. Strong grammar and writing skills are essential for effective communication with clients, colleagues, and other healthcare professionals.

TECHNICAL SKILLS: Candidates must demonstrate proficiency in the Electronic Health Record (EHR) system, telehealth services, and any additional software utilized by the organization, ensuring efficient management of client information. The role requires fine motor skills and dexterity to perform various tasks effectively.

BACKGROUND CHECK: Candidates must successfully pass all federal and state background checks, and their educational and professional history will be verified in accordance with company policies.
