At NorthBay Health, we are seeking a highly skilled and experienced Director of Care Management and Population Health to oversee all aspects of our Care Management and Population Health programs. This includes:
- Utilization Management and Review
- Inpatient and Outpatient Care
- Complex Case Management and Discharge Planning
- Demotion Management and Social Services
The successful candidate will collaborate with our Quality and Operations leadership to support key Population Health quality measures and establish overall direction for our Care Management and Population Health programs. They will implement policies, processes, and procedures to support these programs and provide direct supervision to personnel.
Key Responsibilities- Develop and implement strategies to improve patient outcomes and reduce healthcare costs
- Collaborate with medical staff, nursing, and support staff to ensure effective patient care
- Supervise clinical and professional staff to ensure compliance with regulatory requirements
- Develop and manage budgets to ensure fiscal responsibility
- Stay up-to-date with industry trends and best practices in Care Management and Population Health
- Baccalaureate Degree in nursing or a healthcare-related field
- Master's Degree required
- California Nursing License required
- Certification in Case Management required within 6 months of hire
- Three to five years of related clinical experience in Care Management and Population Health
- Minimum of 3 years of director-level experience in Case Management and Population Health strongly preferred
- Demonstrated initiative, follow-through, and ability to work independently
- Results-oriented management and effective project management skills
- Excellent written and verbal communication and presentation skills
- Advanced computer skills, including spreadsheets, graphs, tables, and reports
- Budgeted salary range: $200K - $230K (annual salary based on years of experience)
- Incentive bonus eligible, up to 15% of eligible annualized salary
- Discretionary, unlimited PTO for Director-level positions
