Clinical Nurse II

As a Clinical Nurse II - Transfer Center Specialist at the University of California, Irvine, you will play a critical role in ensuring the seamless transfer of patients to our esteemed health system. This position requires a high level of assessment and reassessment skills, as you will be responsible for prioritizing and planning the transfer of care for patients who desire to be transferred to UCI Health. You will collaborate with an interdisciplinary team to facilitate, promote, and coordinate high-quality, individualized, safe, and cost-effective patient care. Your ability to advocate for patient self-determination and choice, while practicing cultural competence, is essential in this role. Additionally, you will monitor and coordinate resource utilization throughout the health system and evaluate services needed or being provided by the sending facility. This position requires strong knowledge of Joint Commission, CMS, CDPH, and CCS requirements, as well as excellent written and verbal English communication skills. If you possess a minimum of 5 years of recent nursing experience in Adult ICU or Adult ED, advanced computer literacy, and the ability to manage multiple concurrent projects, we encourage you to apply for this exciting opportunity.
Key Responsibilities:
• Conduct high-level assessments and reassessments of patients who desire to transfer to UCI Health

Requirements:
• Minimum of 5 years of recent nursing experience in Adult ICU or Adult ED

• Advanced computer literacy
• Ability to manage multiple concurrent projects
Preferred Qualifications:
• Unit-specific nursing certification
• Prior transfer center experience
• Minimum of 2 years of recent work experience in case management
• Knowledge of University policies and procedures
• Experience with Epic
• Bilingual skills to communicate effectively with patients and families
Licenses and Certifications:
• RN
• BLS
• ACLS
