MDS Coordinator Job Description

At 600 North Brush OPCO LLC, we are seeking a highly skilled and detail-oriented MDS Coordinator to join our team. As a key member of our nursing department, the MDS Coordinator will play a critical role in ensuring the accuracy and completeness of resident assessments and care plans.

Key Responsibilities:

  • Coordinate the development and completion of resident assessments (MDS) in accordance with current Federal and State rules, regulations, and guidelines.
  • Select the appropriate Assessment Reference Date (ARD) and reason for an assessment to ensure accurate payment by Medicare and Medicaid systems.
  • Participate in weekly Utilization Review (UR) meetings and work closely with therapy teams to ensure accurate information is obtained and goals are met.
  • Ensure timely electronic submission of MDS to the state database and review validation reports to make necessary corrections.
  • Review preliminary reports and ensure timely follow-up action is taken to make corrections.
  • Maintain the Facility CMI tracker and review medical record data to ensure accurate diagnosis codes are entered.
  • Complete PDPM grouper for new Medicare admissions and maintain CASPER login to monitor updates and communicate with administrators.
  • Ensure physician certification for Medicare admissions and work with facility consultants to ensure accurate MDS capture.
  • Maintain a good working relationship with the Business Office Manager and Billing office to ensure accurate communication regarding billing information.
  • Participate in facility surveys and analyze weekly QI and QM reports to identify areas for improvement.
  • Implement and maintain the Kardex for all residents and ensure quarterly and annual resident assessments and care plan reviews are completed on a timely basis.
  • Coordinate the review and revision of resident care plans by the interdisciplinary team and ensure that each portion of the assessment is signed and dated by the person completing it.

Requirements:

  • Strong understanding of MDS and care plan development principles.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong analytical and problem-solving skills.
  • Proficiency in CASPER and other relevant software systems.
