Job Summary

We are seeking a highly skilled MDS Coordinator to join our team at 600 North Brush OPCO LLC.

The successful candidate will be responsible for conducting and coordinating the development and completion of the resident assessment in accordance with state and federal regulations.

Key Responsibilities:

  • Coordinate the development and completion of the resident assessment (MDS) in accordance with current Federal and State rules, regulations, and guidelines.
  • Select the appropriate Assessment Reference Date (ARD) and reason for an assessment to ensure accurate payment by Medicare and Medicaid systems.
  • Participate in weekly Utilization Review (UR) meetings and work closely with therapy to ensure accurate information is obtained and goals are met.
  • Ensure timely electronic submission of all MDS to the state database and review validation reports to make necessary corrections.
  • Review preliminary reports and ensure follow-up action is taken to make corrections within a timely manner.
  • Maintain the Facility CMI tracker weekly and review medical record data to ensure all active diagnosis codes are entered.
  • Complete the PDPM grouper for all new Medicare admissions and maintain CASPER login to monitor updates and communicate with the administrator.
  • Complete and ensure physician certification for Medicare admissions and work with the facility consultant to ensure accurate MDS capture.
  • Develop and maintain a good working relationship with the Business Office Manager and Billing office to ensure accurate communication regarding information for billing purposes.
  • Participate in facility surveys made by authorized government agencies and analyze weekly facility QI and QM reports to ensure corrective action is implemented.
  • Be actively involved in the facility's Quality Assurance and Performance Improvement (QAPI) process and maintain and update written policies and procedures related to the MDS and care plan.
  • Provide and document ongoing in-service training for all staff related to resident ADLs and develop preliminary and comprehensive assessments of the nursing needs of each resident.
  • Ensure that all members of the assessment team are aware of the importance of completeness and accuracy in their assessment functions and that they are aware of the penalties for false certification.
  • Coordinate the development of a written plan of care for each resident that identifies problems/needs, indicates care to be given, goals to be accomplished, and which professional service is responsible for each element of care.
  • Ensure that the care plan includes measurable objectives and timetables to meet the resident's medical, nursing, and mental and psychosocial needs as identified in the resident's assessment.
  • Ensure that an initial resident assessment is completed within fourteen days of the resident's admission and a comprehensive care plan is completed within twenty-one days of the resident's admission.
  • Assist the nursing staff in encouraging the resident and their family to participate in the development and review of the resident's plan of care.
  • Develop the schedule of activities required for the resident assessment and plan of care and assist the Director of Nursing Services and relevant directors/supervisors of other departments in ensuring that all personnel involved in providing care to the resident are aware of the resident's care plan.
  • Ensure that quarterly and annual resident assessments and care plan reviews are made on a timely basis and implement and maintain the Kardex for all residents.
  • Coordinate the review and revision of the resident's care plan by the interdisciplinary team after each quarterly review or other assessment, assuring that the care plan is evaluated and revised each time an assessment is done or when there is a change in the resident's status.
  • Inform all assessment team members of the requirements for accuracy and completion of the Resident assessment (MDS) and ensure that each portion of the assessment is signed and dated by the person completing that portion of the MDS.
  • Sign and date the assessment instrument to certify its completion and ensure that there is a working 802 and 672 available at all times.

Requirements:

  • High school diploma or equivalent required; associate's or bachelor's degree in a related field preferred.
  • Minimum of one year of experience in a long-term care setting, preferably in a coordinator or specialist role.
  • Strong knowledge of Federal and State regulations related to resident assessment and care planning.
  • Excellent communication and interpersonal skills, with the ability to work effectively with a diverse team.
  • Ability to analyze data and make informed decisions, with a focus on quality and compliance.
  • Proficiency in electronic medical records and other software systems used in the facility.
