Join Our Team as an MDS Coordinator

We are seeking a highly skilled and experienced MDS Coordinator to join our team at The Laurels Of West Columbus. As an MDS Coordinator, you will play a critical role in ensuring the highest quality of care for our residents.

Responsibilities:
  • Conduct thorough and accurate assessments of residents to identify their care needs and develop individualized care plans.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
  • Stay up-to-date with the latest regulations and guidelines related to the Resident Assessment Instrument (RAI) process and Prospective Payment Process (PPS).
  • Provide education and training to staff on RAI and PPS best practices.
  • Participate in quality improvement initiatives to enhance the overall quality of care.
Requirements:
  • Current LPN licensure.
  • AANC certification a plus.
  • Experience as an MDS Nurse.
  • Knowledge of RAI and PPS processes.
