We are seeking a highly skilled and dedicated MDS Coordinator to join our team at Providence Care Center. As a key member of our leadership team, you will play a vital role in fostering a culture of compassion, inclusion, and excellence in care delivery.
Key Responsibilities:- Conduct and coordinate the development and completion of resident assessments (MDS) in accordance with current rules, regulations, and guidelines.
- Ensure accurate and timely completion of the MDS process, collaborating with the interdisciplinary team to develop individualized resident care plans.
- Monitor and document resident progress and responses to care plans, participating in the Quality Assurance and Performance Improvement (QAPI) process.
- Communicate effectively with residents, families, and other healthcare providers to coordinate optimal care delivery.
- Stay updated on MDS regulations and guidelines to ensure compliance.
