Community Support Facilitator Job Description

Hope Services is seeking a Community Support Facilitator to provide support and leadership for consumers to participate in activities and receive services in community settings. The ideal candidate will possess a High school diploma, GED, or equivalent, plus two years of related experience or an approved combination of experience and education.

Key Responsibilities:
  • Provide group and/or individual support in the community as identified in the consumer's Individualized Personal Plan.
  • Develop opportunities for clients to participate as valued and contributing members of the community.
  • Facilitate and model interactive relationships with community members.
  • Travel with and/or transport client throughout the local community.
  • Act as advocate on behalf of consumers served.
  • Complete required forms and accountability documentation.
Requirements:
  • High school diploma, GED, or equivalent.
  • Two years of related experience or an approved combination of experience and education.
  • Valid California driver's license and clear driving record.
