Position Summary:
As a Licensed Practical Nurse (LPN), you will work under the supervision of a Licensed Professional (such as a Physician or Nurse Practitioner) to deliver high-quality patient care. Your responsibilities will include utilizing the nursing process to assess patient needs, prioritize care, and evaluate outcomes.

Key Responsibilities:
  • Conduct thorough assessments and collaborate with healthcare providers to create and maintain individualized care plans.
  • Provide educational support to patients, families, and team members in coordination with other healthcare professionals.
  • Facilitate the coordination of patient care among various healthcare entities according to established care plans.
  • Administer medications safely and in accordance with organizational policies and state regulations.
  • Serve as a clinical mentor and subject matter expert within the practice, particularly in lead roles.

Qualifications:
  • Current Licensed Practical Nurse (LPN) license in the state of employment or LPN applicant status (required).
  • BLS Basic Life Support certification from the American Heart Association (required).
  • Preferred: 1 year of clinical patient care experience.
  • Preferred: Training in EPIC Electronic Health Record (EHR).

Essential Skills:
  • Ability to take and document vital signs accurately.
  • Experience in patient rooming and monitoring during treatments.
  • Proficient in documenting medical records and managing patient communications.
  • Strong critical thinking and communication skills.
  • Demonstrated ability to work collaboratively in a team environment.
