Key Responsibilities- Assessment and Care Planning: Conduct thorough assessments of patients' conditions, identify priority needs, and develop and implement individualized care plans in collaboration with healthcare providers.
- Patient Education and Support: Provide education and support to patients and their families, promoting health literacy and empowering them to manage their care.
- Care Coordination: Coordinate patient care with other healthcare professionals, ensuring seamless transitions and optimal outcomes.
- Medication Administration: Safely administer medications in accordance with organizational policies and procedures, as well as state requirements.
- Leadership and Mentorship: Serve as a subject matter expert and clinical mentor, providing guidance and support to colleagues in a lead role.
- Licensure and Certification: Current Licensed Practical Nurse (LPN) licensure from the state of employment or LPN applicant status.
- Education and Experience: 1 year of clinical patient care experience preferred.
- Skills and Abilities: Strong communication and interpersonal skills, ability to work effectively in a fast-paced environment, and proficiency in electronic health records (EHRs).
This role may involve periods of high stress and fluctuating workloads, exposure to physical altercations and verbal abuse, and potential exposure to hazardous substances or body fluids. The ability to exercise universal precautions, use personal protective equipment, and adhere to infection control policies is essential.
Physical RequirementsThis role requires manual dexterity, ability to perform shift work, and exposure to various physical demands, including lifting, carrying, pushing, and pulling. The ability to maneuver patients, hear alarms and telephone audio, and reach above shoulder level is also required.
